Family Genogram or Ecomap or Attachment Diagram: include ONE of these
(your option to choose which) with each summary journal for each
family you followed.
Answer the following questions briefly.
Assessment & Plan (Related to the clinical objectives #1, 2, 4, 6, 7)
- What did you see as issues for the family? (Priority based on your assessment)
- What did the family perceive as issues for the family (Priority based on the family's
perception)?
- Were there differences in perception of the issues between you and the family?
- Describe or list the family strengths
- Derive nursing diagnoses for this family (or OMAHA's classification system).
- What goals were established mutually with the family? How?
Intervention (Related to the clinical objectives #3, 4, 5, 7, 8)
- Describe your teaching activities during the family visits.
- Describe intervention methods you frequently used.
- What in the situation would facilitate your interaction or intervention with the family?
- What in the situation would hinder your interaction or intervention with the family?
Evaluation (Related to the clinical objectives #9, 10)
- Evaluate your family visits from your perspective.
- Evaluate your family visits from what you believe to be the family's perspective.
- Evaluate your learning activities directed toward meeting self-identified learning needs.
- Discuss need for change in health care to more effectively meet family health needs (if applicable)
Directions
- Page limit: 3- 5 pages for each summary journal.
- One Summary Journal for each family followed.
- Attach the Clinical Self Evaluation Sheet, with letter grade, to the Journal Summaries (20 points).
The Clinical Self Evaluation covers BOTH families followed.
