Goals: (Write one to three for the lesson. They are statements of broad general
outcomes of the instructional focus of the lesson.)
Objectives: (Write one to three objectives in each of the three
categories below. They are specific statements of learning outcomes, describing
what it is the learner can actually do after the lesson. Objectives are
expressed from the learner's point of view.)
1. Cognitive Objectives: (Objectives that deal with intellectual processes.)
2. Affective Objectives: (Objectives that deal with emotions.)
3. Psychomotor Objectives: (Objectives that deal with movement skills and
processes.)
Equipment: (List and describe any and all equipment used in the
lesson.)
Safety Precautions: (List and describe any special concerns for safety
in this lesson that should receive special attention.)
References/resources: (List the resources that were utilized for this
lesson.)

Format Description
Part One: Focus and review
• Focus the learner's attention
• Relate previous learning
• Diagnose prerequisite skills
• Give reason why lesson is important
Part Two: Statement of Objectives
• Let students know exactly what is
expected
• Alerts students to essential learning
Part Three: Teacher Input
• Teaching - what the teacher does.
(Presentation of learning activities)
• Input has many forms
• Input includes - checking for
understanding, monitoring and adjusting
Part Four: Guided Practice
• Close monitoring of all students' first
attempts at new learning
• Prevents students from practicing errors
Part Five: Independent Practice
• Practice new learning alone
• Internalize new skills
• Acquire speed and fluency
Part Six: Closure
• Encapsulates lesson being taught
• Summarizes major concepts
• Allows learning to be expanded with later
lessons of recalled for application
Note: Each of the six parts is
subdivided into four components
1. Time: This component provides the reader/user of the
lesson with the approximate amount of time allocated to each segment of the
lesson
2. Procedure: This component provides the reader/user of the
lesson with a description of the actual procedure(s) that will be utilized to
implement each individual part of the lesson itself.
3. Formation: This component provides the reader/user of the
lesson with a drawing/description of the actual formation that is to be
utilized for teaching each portion of the lesson.
4. Teaching Points: This component provides the reader/user of the
lesson with a description of the main teaching points/cues to be utilized for
the teaching of skills throughout the lesson.
